K2 Committee Evaluation of the Causes Failure after Baseform Decrease when Casting Top Slab Box Traffic Ramp 8 in Depok - Antasari Toll Road
Jakarta - There was a decrease in the west side baseform when casting the Top Slab Box Trafic Ramp 8 Krukut Junction in the Depok - Antasari (Desari) Toll Road in the Krukut region, Depok, West Java, on Tuesday, October 8, 2019 at 01.42 WIB in the morning. There were no fatalities in this incident.
The baseform decline occurred during casting work with a total volume of 792 m3 from the east and west sides. At the time of casting there was around 180 m3, causing a decrease in baseform on the West side.
Head of the Toll Road Regulatory Agency (BPJT) of the PUPR Ministry, Danang Parikesit accompanied by the Head of BPJT Engineering Primawan Avicenna in the morning immediately checked into the scene.
"The decrease in baseform is caused by the lack of strength of the scaffold to hold the weight of the concrete being casted. Then at this time, around the location has been carried out cleaning material, "said Danang in Jakarta, Wednesday (9/10/19).
The Ministry of Public Works and Housing (PUPR) instructed the Project Leader of PT. Citra Waspphutowa (CW) Desari Toll to temporarily stop casting activities and sterilize the work area.
The Construction Safety Committee (K2) team and the Road and Bridge Tunnel Safety Commission (KKJTJ) this afternoon have carried out investigations in the field and instructed to stop the box traffic casting activity.
Work can continue after the revision of the shoring / scaffolding design for Ramp 8 and the shoring / scaffolding reinforcement design for Ramp 5 are submitted, inspected, and approved by the Construction Safety Committee.
BPJT Ministry of PUPR has asked PT. Citra Waspphutowa (CW) as the Toll Road Business Entity (BUJT) to implement the Occupational Safety and Health Management System (SMK3) that has been regulated in Minister of Public Works Regulation Number 5 / PRT / M / 2014 concerning Guidelines for Occupational Safety and Health Management System (SMK3) ) Construction of the Field of Public Works.
Internal supervision from BUJT on the implementing contractor must also continue to be carried out more stringently in the implementation of toll road construction activities in order to avoid recurrence of similar incidents.
